Miami-Dade County Public Schools (M-DCPS) continues to offer Retirees and their eligible dependents the below Medicare healthcare plans and Prescription Drug Plans (PDP).

• Nine Medicare Advantage Plans:

♦ Four AvMed (Medicare Choice HMO* (Miami-Dade County only and Broward County only) and Medicare Circle* (Miami Dade County only and Broward County only) plans

♦ Cigna Leon Cares

♦ Two Humana (HMO* and PPO) plans

♦ Two UnitedHealthcare (UHC) plans

• Three UHC Medicare Supplement Plans: A, F & N
• Four UHC Part D Pharmacy Plans: Comprehensive, Preferred, Premier and Saver Plus

*NOTE: At the time of enrollment, a Primary Care Physician (PCP) is required for the AvMed Medicare Choice HMO, AvMed Medicare Circle HMO and the Humana Zero Premium HMO plans.
