Part-time (BEFL) Employee Benefits 

Open Enrollment Facts

Davis Vision WILL NOT be offered for the 2019 plan year. If you are currently enrolled in the Davis Vision plan, your coverage will terminate on December 31, 2018. We will continue to offer vision coverage through UnitedHealthcare. If you do not re-enroll during this Open Enrollment period the following will occur: 

  • Your current healthcare coverage will continue; plan design changes will automatically be adjusted. 
  • Your dependent(s) healthcare coverage will continue, plan design changes will automatically be adjusted.
  • Yours and your dependent(s)’ flexible benefits will continue, with the exception of Davis Vision; premium changes will automatically be adjusted.
  • You will be receiving monthly billing invoices reflecting your 2019 benefits.

You may select from three Cigna healthcare plans. Dependents must be covered under the same plan as the employee.
The healthcare plans are:

  • Cigna Open Access Plus (OAP) 20
  • Cigna LocalPlus Plan
  • Cigna Open Access Plus (OAP) 10*

NOTE: * This plan is only offered to currently enrolled employees. This plan is not offered to employees represented by the AFSCME Union.

To enroll in a Cigna plan, effective January 1, 2019, you must request an enrollment form. If you are adding dependents, you must include your dependent’s Social Security number on the form and submit dependent eligibility documentation (e.g. marriage certificate for a spouse and birth certificate for a child, etc.). Mail your completed form, with your first month premium, payable to The School Board of Miami-Dade County, Florida to:

Note: There is no Board contribution toward the employee’s or dependent(s) coverage. Part-time employees are responsible for the full, monthly cost of the Board-approved healthcare plans.

Flexible Benefits

If you are currently enrolled in any of the flexible benefits, you do not need to submit an enrollment form. Your benefits will continue, with the exception of Davis Vision. NOTE: There are no plan design or premium changes to these plans.

To enroll in any of the flexible benefits, effective January 1, 2019, you must request a flexible benefits enrollment form. Mail your completed form, with your first month’s premium, payable to FBMC Benefits Management, Inc. to:

Note: If you choose not to enroll in one of the disability plans (Short-Term Disability or Long-Term Disability) at this time, future enrollments will require that you complete Evidence of Insurability (EOI) before you are considered for coverage.

Dependent Coverage:

  • Dependent Social Security numbers are required during open enrollment. If your dependent’s Social Security number is not provided, coverage for the dependent cannot be processed. For additional information, call 1-855-MDC-PS4U (1-855-632-7748).
  • Documentation of your dependent’s eligibility must be provided. Eligibility documentation requirements can be found in the Dependent Information page.
  • Children may include: natural-born children, stepchildren, adopted children and children for whom you have been appointed legal guardian. Your unmarried children are eligible from birth until the end of the year in which the child reaches age 26, if the child is:
  1. Dependent on you for support; or
  2. Lives in your household; or
  3. Is enrolled full-time or part-time in an accredited school, college or university. See more information in the Dependent Information page.
  • Children of your domestic partner are eligible for coverage only if the domestic partner is also included in the coverage.

What is the Open Enrollment Period?

The Open Enrollment period is a period of time, determined by your employer, during which you are allowed to make any changes to your current benefits.

When are benefits for the new plan year effective and for how long?

The benefits are effective January 1, 2019 through December 31, 2019.

Do I need to enroll?

No. This is a changes only enrollment. However, Davis Vision WILL NOT be offered for the 2019 plan year. If you are currently enrolled in the Davis Vision plan, your coverage will terminate on December 31, 2018. We will continue to offer vision coverage through UnitedHealthcare.

If you are not making any changes to your current healthcare and/or Flexible Benefits, you do not need to return an enrollment form. Your current coverage and your dependent(s)’ coverage will continue, with the exception of Davis Vision. Healthcare plan design changes and Flexible Benefits premium changes will automatically be adjusted. The cost of these benefits will not be deducted from your paycheck. Payment coupons will be automatically sent for your monthly premiums.

What changes can I make during this enrollment period?

You could change your current medical plan, add an eligible dependent or delete a currently covered dependent.

How do I enroll?

Your open enrollment will take place on paper. You must request, complete and return the form by December 11, 2018.

Can I enroll in Accidental Death & Dismemberment Coverage?

Yes, except if you are represented by the AFSCME labor union. AFSCME employees CAN NOT enroll in this benefit.
